Family Practice Exam: Drugs, Drugs, and More Drugs

Today (April 15, 2016) I sat for my family practice board exam–for SEVEN hours.   That will be the last time I put myself through something as ridiculous as that endeavor!

I have been Board-Certified in family practice since I graduated from my residency (with a short period where I missed taking the exam).  I last recertified 10 years ago and when my letter came informing me that I would have to sit for another exam, I debated whether to do it.

You see, over the last twenty years or so, I no longer practice conventional family practice. Now, I practice holistic medicine.  I still use much of what was taught to me in my training, but now, I primarily rely on items that support the body’s physiology and biochemistry and I strive to avoid using items that poison and block the body’s pathways.  Unfortunately, nearly all of the drugs currently used in conventional medicine work by disrupting the body’s physiology and biochemistry by poisoning enzymes and blocking receptors.  I have written about this extensively in my books, Drugs That Don’t Work and Natural Therapies That Do and The Statin Disaster.

In my SEVEN (miserable) hour board exam today, I found myself getting more and more irritated.  Nearly every question was about a drug—how it is used, the side effects associated with it, and how it interacts with other drugs.  There were virtually no questions about the importance of diet or the importance of identifying nutritional deficiencies.  In fact, I can’t recall one question along those lines.  The patient vignettes that I suffered reading through were all the same—the patient was taking multiple drugs and I was asked one obscure question about the drug therapy prescribed to the patient.   I actually felt bad for the poor patients in the vignettes as they were having their biochemistry poisoned with multi-drug prescription cocktails.

If this is what it takes to be board-certified by the American Academy of Family Practice (AAFP), I say “Fugetaboutit”.  I will not suffer through this nonsense again. 

I am amazed that the AAFP does not even acknowledge the importance of diet and nutritional support in their exam.  Yes, there may have been a few token questions (but I can’t recall one), but they were few and far between.  I guess the AAFP believes that prescription drugs can cure all that ails.  It is too bad that the medical research does not support this idea. 

Folks, we spend nearly 20% of our GNP on health care.  That is well over twice the average for all the other major Western countries.  What do we get for that big expense?  We get more disease, increased mortality rates in infants and adults and more chronic illnesses. And, of course, we get lots of prescription drugs that poison enzymes and block receptors.  The Big Pharma Cartel couldn’t be happier. 

The AAFP and the rest of medicine needs to change.  I felt sorry for my young conventional colleagues who were taking the exam.  They were taught virtually nothing about what health is and how to promote and maintain it.  Unfortunately, the only tools in their toolboxes are prescription drugs.    

SEVEN hours taking a ridiculous exam?  That will not happen again.

    It was important for you to take that test, just so that you could report back to us how bad conventional medical training is … and how terribly off course today’s conventional doctors are equipped in regards to getting people back on track to good health. The AMA and their medical schools are just spitting out doctors who will do us much harm, contrary to the oath that they must make, …”to do us no harm.” ObamaCare was created to force people to pay into a dreadful ‘unhealthful’ insurance system that supports Big Pharma’s cartel. I have always wondered why doctors, such as yourself, and other types of physicians have not banded together to create health insurance choices for us that will allow us to utilize your services. This is what it will take to turn the system around! People want healthful, helpful choices. So far, we haven’t had those kind of choices available to us in health insurance. Please band together with other types of health providers and make that happen for us!

    It is truly wonderful to see someone break free of the intense brainwashing that medical doctors go through. Conventional medicine does not even deserve to be called a form of medicine, it’s an anti medicine actually, and this includes their recommendations as well. We need to defend ourselves from all of this and keep up the good work helping people do exactly that!

    Dr B, Thank you for educating the common folk like me about a) what I can expect an AAFP practitioner to be focused on, not to assume his or her AAFP certification makes a more functional medicine-centered doctor, and b) the need for any individual patent to self educate in order to be healthy.     As we know, pain meds classes are required for continuing education. Too bad diet, healthy life style and fitness aren’t part of the program. Oh wait, that would be too easy. One colleague to another, we do the best we can and keep on learning.

    Dr B – I couldn’t agree with you more, but think of it this way– How can a doctor like yourself help a new patient SAFELY wean themselves off of their more dangerous prescription drugs?

    Yes, drugs are a drag, but if YOU don’t know what you’re dealing with, then you’ll have to engage a conventional doc or pharmacist who does know. And that’s not to mention that some patients, who are new to natural medicine, are sometimes hesitant to quit their meds “cold-turkey.”

    It’s time for some name changes.
    Medicine is autonomic. The science practice, diagnosis and treatment of disease. A far cry from the etymology.
    Doctoring is autonomic. Treating people who are ill.
    The pharmaceutical companies are not motivated to effect a cure as, very quickly, they would run out of revenue! So their targets are the very people who are defined by their titles. (If their thinking isn’t critical enough to spot what is going on) Perhaps we should rename this group as zotologists.

    It is a great shame that the ‘alternative’ label has been denigrated and ridiculed almost beyond acceptance, even elevating the sheeple of the world to be experts, regurgitate theIr eye rolling potted versions of received wisdom.

    Maybe the ancient Greeks could provide a revision. Cures and healing as dignified by the Goddess Iaso. Iasotherapist.

    Thank you Dr. B for your insightful post, how agonizing it must be for someone such as yourself, holistically, organically, and true health oriented, to have to answer questions about drugs tyat you have no desire to prescribe, or inflict upon your patients. I’ve learned so much from your blog, and being hypothyroid myself, have researched as much as possible about how to treat my condition naturally and holistically. At 62, I haven’t been to a doctor in many years. I order my natural thyroid supplement overseas, take iodine and DHEA, and other whole food supplements to support my adrenals, hormones, and endocrine system. I have no desire to visit a mainstream doctor since all they want to do is give me Rx drugs, and constantly remind me that my life is in jeopardy if I don’t take them. My husband himself has really been through the ordeal with Rx drugs after his heart surgery, and has been gradually weaning himself off of all drugs, and he refuses to take the statin drugs they insist he take. He’s fired 4 different cardiologists because they are all STUPID and sold out to the ‘system’. He’s doing really well now, and realizes that he does NOT need those drugs at all!!

    We believe in what Hippocrates said: “Our food should be our medicine and our medicine should be our food.”

    I can see why such an exam would drive you nuts. But I can also see why doing it, even if you hated it, gives you far more credibility to say what you do. The whole drug scene changes so dramatically so quickly, that after 10 years, drug prescribing and “rationale” can change hugely. To be able to say, “I was a good parrot and got 95% in an exam about drugs I will rarely use, gives me the right to speak out, as to why I will not use them”

    You may see it as a waste, but if I came to you, I would consider the fact that you passed that exam, valuable, because I would know that you understood the crap Big Pharma put out, and therefore would understand better the damage it was doing to me, and how to possibly fix it. If I came to you, on some new drug and you had no idea about it, yes, you could look it up, but having done that exam, you’re saying, “I don’t like what you are doing, but I’m keeping up with it, so that I have the right to editorialise on you, without you saying that I’m a dinosaur.”

    So please, never say never. In nine years, reconsider, and then decide whether or not you want to put your head through the wringer.

    There is also the other point too, that the alternative practitioner groups also want you to certify, so I can understand why you could feel that you have to choose.

    To me, it’s a point of value that you have done this, and when you’ve got your certification, then put if prominently on your website to let the world know that you gave the answers they wanted, they have no choice to certify you, but you believe in healing, not making people’s health worse their way.

    I learnt the hard way from these ‘Board Certified Pretenders’ … they are not true ‘Doctors’ because they do not ‘teach Truth’. Most have been corrupted/ deceived by ‘the Root of All Evil’ = ‘the love of money’ {1 Timothy 6:10]
    My wife and myself have five children … they have all grown up now and they have all witnessed a wondrous miracle. When our children were young and as I was driving the Tarago’s around they could read the street signs before me and frequently yell out -> ‘Dad, Dad you need to get your eyes checked’.
    I personally did not ever see a Doctor for myself, I was never sick but complied with the Government’s requirements for their Immunisations. Finally I went and saw the local G.P. whom I thought at the time had proven himself trustworthy with my children. For my fortieth birthday I was informed I had Type II Diabetes and would need to take Medications. I foolishly complied and four years later I needed an Emergency double Bypass. The Drugs kept on coming and my health kept on declining. I was referred to The R.P.H. Diabetic Clinic and remained under their care for fifteen years. Every drug has side effects, -> longer we take them … main effects.
    The Number of drugs all these different Drs.? had me taking was Crazy; my health kept deteriorating rapidly
    We had to sell my ¼ acre block in Maylands because the old asbestos home was too much work and the children were fast growing out of room. Our Children were our priority Boy, Girl, Boy, Boy each 2 years apart then 4 years later another Boy. We bought a newer Home with 2 Bath 4 Bedrooms and a granny flat. On half the size of land.
    At this Time I was not internet savvy and the ‘True Health information’ down under in Australia was none existent ~ ‘in my experience’. Our second Son, our No. 3 child insisted he needed the internet for his academic studies. I was resistant however cautiously complied.
    As my Health continued its spiraling decline; I determined that maybe the internet could help me to Research into my own health. Finally the light and wisdom from my youth began to awaken and as I continued to ‘always dig deeper’ … I began to slowly “One day at a time” … take back the Control to my own Health …
    This was a very long journey after 18 years of misplaced trust, however Now as I Approach my Sixty second Birthday, I have regained my Health and Vitality and have remained totally big Pharmacy ‘Free’ into my fourth year Now. My wife also though hesitating at first wondering whether I was being wise or foolish has now entered her third year also … ‘Free’ from big Pharma’s Pimps with their Toxic pills … for every ill.
    I learnt big “PHARMA CURES NOTHING” they merely carelessly cover up the warning lights our body is desperately sending us

    I read a book “Pure White and Deadly” by Dr. John Yudkin -> He Proved … as our sugar levels increase we become more short sighted.

    Life is Good and we both TRUELY have a Lot to Thank the good Lord for; We thank God daily for His Living Foods

    I have No Chimp D.N.A in my brains ……

    Dr. B, Keep doing what you do. I was diagnosed with a Hiatial (sp?) hernia. And so, my doc gave me Pantoprazole. I read the side effects and I did not take it. After talking to some friends, I decided to at least try it one time. I took the pill yesterday. I woke up this morning and I had a bout of diarrhea that lasted for 3 hours. The skin on the top of my head was pealing. I found the friendly side effects sheet that the Pharmacist gives you. I re-read it and I was mad. No more of that drug. There is no way that I will ever take that drug again. By the way, when the doctors tried to give me a statin drug, I had bad side effects. I take niacin instead.

    Dear Dr. Brownstein,
    When my father was 39, he suffered a devastating heart attack. Everyone was shocked because he was a star athlete & looked to be in his prime. He was given six months to a year. My Italian mother read everything she could get her hands on regarding foods & diet. She totally changed our meals: skim milk, Fleischmann ‘s margarine, fruits, salads, veggies, fish, poultry -broiled or baked not fried. She cooked with herbs & wine. Dad lived almost 20 years, suffering two more small attacks before the last one while he was golfing. My mother is still alive at 105.

    Our healthcare system is seriously corrupted by the pharmaceutical companies and insurance companies. Despite brainwashing from medical literature and drug reps, I was moving toward more holistic treatment when I became disabled and with no job or insurance, had to turn to natural therapies. I got SSDI on the first try because I was in such bad shape but after stopping the drugs and continuing to learn about more natural therapies, my health is much better (reversed diabetes, lost weight, less chronic pain and brain fog). I am now retired and so never going to practice again but I write books to help others. I was fortunate to find a doctor who is willing to go along with what I want, like bioidentical HRT, refusing vaccines etc. I really enjoy reading your blog and am always learning.

    Having recently gotten off crappy bisphosphanates and switched to an alkalizing/plant-based diet with targeted bone support, I’ve come across so many new resources that promote “nature’s pharmacy” and raw-food fueling and being your own health advocate. Unless you’re birthing a baby or pulled out of a car wreck or you need someone to order blood work or a DXA, well – a lot of what medical doctors do these days IS harm.

    Between the AMA, Big Pharma & the HCA – there’s no better reason to educate yourself. Don’t be the next poor patient.

    Dr. B – As a long-time RN, I always had the greatest level of respect for family practice physicians because they practiced more of a common sense, well-rounded style of medicine. My, how times have changed! I remember hearing these physicians recommend fresh air, sunshine and taking an outdoor walk to their patients. I am fortunate to have found a holistic MD in my area. We were discussing thyroid function during an appointment and she said “Are you familiar with what David Brownstein says about iodine?” I let her know I was a fellow subscriber to your newsletter. Keep up the good work – my doctor and I will keep reading.

    It was the “inside view,” from having taught in my Family Medicine residency, that showed me how holistic medicine would not be served by me maintaining my Board certification status. After making my decision last year, I was able to get a refund on the preparatory modules for which I’d prepaid. No more worthless exams, just preparation and learning directed at helping people heal and optimize health.     Thank you for your authenticity, and courage. As I sit, only minutes away from going to a conventional doctor, your words resonate in my spirit. A strong believer in holistic medicine, I am living with pain in my shoulder which is almost unbearable at times. I use Yoga, meditation and natural muscle relaxers to ease the pain. If my MRI shows no torn ligaments or rotator cup….I am out of there like a flash. The western-med “communiy” is not one I care to be a part of.

    Again, thank you. Your blog has given me courage and hope. And, maybe one day I’ll have the pleasure of meeting you, on healthy terms that is.

    The AAFP’s tag line is Strong Medicine for America which is very telling! They accept grant money from Coke justifying that they carry a water line. They push loads of vaccines and statins and they spend a lot of time on initiatives such as concierge medicine as they have a large segment of the membership who complains that they don’t make what surgeons make. The medical associations are very behind the times and definitely not in step with progressive health and wellness. The dental association, ADA, has an entire large department dedicated to fluoride in the water–and lobbyist in DC pushing this agenda. The American Heart Association, not only supports an outdated food pyramid, but not that long ago they didn’t accept Jewish members and that is why the the American College of Cardiology formed in DC. These associations are horrible throwbacks and aren’t doing much to progress medicine!

    I am taking a coarse to become a Nutritional Therapy Practitioner and when I learned of the signs of being B1 deficient (Beri Beri), I became sick to my stomach. The reason being, the symptoms fit my grandson as a newborn and continues as he is 18 months old now. He had the cry (scream) that was described in my book at birth. It was concerning to his Mom at the time, but she can’t even remember it now. He can’t walk and is barely starting to stand. It seems obvious to me he is deficient in B vitamins and especially thiamine. I am shocked this hasn’t even been addressed with the parents until now at his 18 month appointment where the doctor finally suggested that they take him to a nutritionist because he is below the lowest percentile in weight. His mother was in denial that he needed to go to a nutritionist because she said that he was underweight because he refused to eat the few weeks prior to the appointment with the doctor. Thiamine deficiency can also cause a poor appetite.

      I routinely check thiamine levels on my patients. I would estimate that thiamine deficiency is present in 25% of my patients.

I used to review the exam board questions long ago…usually answers are based on Big Pharma’s research……and the last time I took the exam I was disgusted that I had to answer what they wanted…. but the correct response is usually not even offered….because they are not considering lifestyle, nutrients, toxins and more.

    So sorry you had to waste seven grueling hours on a Big Pharma Test. I have a few questions about not taking the recertification exam. Does that mean you will no longer qualify as a board certified family physician? If you want to practice as a holistic doc, do you need more specialized schooling?

    I was attending a conference today and thought about you, because one of the speakers was saying how much opposition she gets in regard to her work and how much education there still needs to be done around cholesterol & fats in general.     Dr B here is a novel idea. Perhaps you could attempt to educate those formulating the exam? Would be an uphill battle for sure, but one never knows. At least maybe they would hear you out and have something to think about, since they are unable to do so on their own. That was very educational to read about and thank you.

When you point out the poisonous drugs and vaccinations of modern medicine, it holds more weight because you are one of ‘them’ as far as your education. Modern medicine seems to be managing disease rather than looking for causes and cures. The powers that be are not happy with the truth that you speak. I have four of your books.     That sounds grueling! I’m a licensed ND, and we have a lot of pharmacology questions on our board exams, too. I’m so grateful that I discovered functional medicine early in my career and never applied to conventional medical school. My surgeon father fully supported my choice. The last line of your post hits the nail on the head: The only tools in their toolbox are prescription drugs. I love what I do because my toolbox is full of clinical nutrition, botanical medicine, physical therapy, hydrotherapy, counseling, lifestyle prescriptions, homeopathy, AND pharmaceuticals plus minor surgery when necessary! I consider it malpractice to treat a patient with drugs while doing nothing to actually support the foundations of health. It’s doing harm by “omission” rather than “commission.” Keep doing what you’re doing, Dr. B.; we need more docs like you!

    This is a heartbreaking situation when you have to personally witness every member of your family’s life being horrifically negatively impacted and destroyed and you know that big pharma and the “doctors” are ultilizing them as cash cows, causing horrible suffering, gaining monetarily off of it and having zero awareness or accountability every step of the way. The situation with my Dad (who they butchered and then poisoned to death)!! actually contributed to MY getting sick and no one in my family believes in aternative healing. I am totally broken hearted over this and cannot make peace with it. To add insult to injury my mother actually donated over 10 thousand dollars to a parasitical “non- profit” called Lustgarden who claim to be doing research to find a cure for pancreatic cancer even though the cancer started in his stomach or duodenum!!! The whole scenario is indicative of how sick our entire culture is and how evil the medical system is. I consider it akin to a covert holocaust. John Mayer has a sing about how he and his friends are waiting for the world to change. Well folks I’ve gotten used to it for you it doesn’t change until you do. It’s up to every single one of us to do our part show up stand up to speak up and to say no even if you feel like a voice in the wilderness it’s not true they’re listening and even if they are listening but they don’t hear you you’re planting seeds for the Next Generation. Like Gandhi said: never give up and fight the good fight!! God is watching you Dr. Brownstein and you’re certainly giving him a good show. Trusting what you know is the truth and having the courage to speak it and being an awesome role model for other “doctors” is very admirable indeed. Thank you for writing the Forward for Lynne Farrow’s book called The Iodine Crisis which is literally saving my life and no doubt many others. GOD BLESS YOU AND HER!     Hi, Maybe they would not let a person pass who wrote that natural help is better and helps the root cause without side effects. What would a Naturopathic test look like? A Lyme doctor may help Lyme better with natural herbs/homeopathics and could still have the same office helping the same people. Get a 2nd license for natural help? Doctors should not need to be pharmacists, but giving old people 4 or more medicines to cover symptoms just lets the person get worse since diet/life style etc is not helped usually. Keep writing your awesome books! They are great help! God bless!

    What a waste of time!

    With all you have published and promulgated, the notion you need some external authority like the American Academy of Family Medicine to certify you is pretty absurd. You should be certifying them, not the other way around!

    It’s exactly the same in dentistry – All new students learn is how to fix and fill.

    Sadly there are almost no dentists working to avoid filling and fixing teeth – working to promote pristine oral health….
    Weston Price would turn in his grave…..I feel your pain.

    I am sorry for your ordeal! What an enlightening experience! It just confirms what we already know… That conventional medicine is way off target and more and more people are going to be injured or worse! I love what you do so thank you for everything you do and keep on educating and informing the right way to practice medicine! Namaste!
