When to increase just depends on how you feel, i would go for at least few days with only .25
imo its not necessary to take more then 0.5mg at once.
for mixing the vial add 2 ml so every 10units = 0.5mg on most(not all) insulin seringe 10unites is divided in 5 stripes so every stripe is 0.1mg (when added 2ml of bacwater)
yes you should freeze the unmixed vial, the best way is to prepare all your needles and freeze them too so you dont have to keep a vial refrigerate.
The mt1 rates recommended by Cees are a bit low in my opinion for a red head who may well genetically have a naturally higher resistance to the drug due to receptor site mutations.* I would suggest starting at .5 mg ED, progressing to 1mg, or even 1.5 mg.*
.5 mg/d for 2 days to check for interactions
1mg/d there after for a total of 30mg.* If you can see good results continue at 1mg
If results are not forthcoming, 1.5 mg/d for another 20mg
If results are still not acceptable, you may have a high resistance and need to switch to mt2. You will know by 50mg in if the more subtle effect of the mt1 is for you.
I would suggest 1ml of bac water per 10mg vial as outlined by Cees.
